CareAssist may collect information you provide through our website, forms, phone calls, email, text messages, service inquiries, scheduling, and other communications. This may include name, address, email, telephone/mobile number, service information, communication preferences, and information needed to provide or coordinate services. Where applicable, we may collect protected health information (PHI) as part of our services.

We collect information directly from you, from authorized representatives, from caregivers and service interactions, and through website technologies. CareAssist maintains privacy and security safeguards for PHI consistent with applicable HIPAA requirements. PHI is used and disclosed only as permitted or required by applicable law and our Notice of Privacy Practices, where applicable. Our website and ordinary SMS communications are not intended for the transmission of sensitive medical information or PHI.
Please do not send medical information, diagnoses, treatment details, Social Security numbers, or other sensitive information by ordinary website forms, email, or SMS unless CareAssist has specifically instructed you to use an approved secure method.
4. SMS Communications
SMS Consent and phone numbers collected for SMS communication purposes will not be shared with any third party or affiliates for marketing purposes.
If you opt in, CareAssist may send service-related messages such as appointment reminders, confirmations, schedule notifications, follow-ups, caregiver/service coordination, customer support, and billing notifications where applicable. Promotional messages may be sent only where permitted and properly consented to. Message frequency may vary. Message and data rates may apply.

5. Sharing of Information
CareAssist does not sell personal information. PHI is shared only as permitted or required by applicable law. SMS consent and mobile numbers collected for SMS purposes are not shared with third parties or affiliates for their marketing purposes.

Information may be retained as needed to provide services, maintain business and legal records, meet contractual or regulatory requirements, resolve disputes, and protect legitimate business interests.
